Common information

rivastigmine 4.5 mg

Based on evidence from animal studies the major cytochrome P450 isoenzymes are minimally involved in rivastigmine metabolism.

Patients who develop application site reactions suggestive of allergic contact dermatitis to rivastigmine patch and who still require rivastigmine treatment should only be switched to oral rivastigmine after negative allergy testing and under close medical supervision.

As a carbamate, rivastigmine binds to AChE which cleaves the rivastigmine molecule, releasing a phenolic cleavage product that is almost pharmacologically inert and is rapidly excreted via the kidneys.

Therapy with rivastigmine should only be started if a caregiver is available who will regularly monitor intake of the medicinal product by the patient. The curves demonstrate that both patients assigned to EXELON and placebo have a wide range of responses, but that the EXELON groups are more likely to show the greater improvements. A similar European trial in 725 patients with mild to moderate AD that compared 6-12 mg day to 1-4 mg day of rivastigmine and placebo, also for six months and using the same outcome measures, had similar results.

As the disease advances, people may not know what drugs are for or even what they are. Although not observed in clinical trials of rivastigmine, drugs that increase cholinergic activity may cause urinary obstruction.

At six months, treated patients improved modestly on cognition, overall function, and psychiatric symptoms. The 6 mg to 12 mg per day group was statistically significantly superior to placebo, as well as to the 1 mg to 4 mg per day group. Patients who show signs or symptoms of dehydration resulting from prolonged vomiting or diarrhoea can be managed with intravenous fluids and dose reduction or discontinuation if recognised and treated promptly.

  • If doses are delayed, missed, or not taken as prescribed, levels of AZT in the blood may fall too low.

Patients taking diuretics with ziconotide may be at higher risk of depressed levels of consciousness.

Your doctor may advise you to stop taking it 24 hours before surgery.

Increases in blood urea and plasma creatinine, hyperkalaemia reversible on discontinuation may occur, especially in the presence of renal insufficiency, severe heart failure and renovascular hypertension.

Of cases with known outcome, most patients recovered and in cases with a fatal outcome, the majority involved multiple drug ingestion.

This can put you at higher risk of heart attack and stroke.
